Abstract

ABSTRACT Half of the patients with Heart Failure (HF) have a preserved left ventricular ejection fraction (HFpEF). Morbidity and mortality of HFPEF is almost the same as that of HF that reduced ejection fraction, although effective treatment is still unclear. The diagnosis of HFpEF is often very challenging and based on clinical evaluation, echocardiography and other. Exercise training has been shown to improve the quality of life of patients with systolic heart failure, and as an adjunctive therapy of heart failure that recommended by American or European cardiology associations. Most exercise training studies use heart failure patients with decreased ejection fraction, many mechanisms can be studied in looking at improving the quality of life of patients, but data on the role of exercise training for heart failure patients with preserved ejection fractions is still very limited. Abstract

ABSTRAK Dilated Cardiomiopathy (DCM) merupakan penyakit pada otot jantung, yang ditandai dengan dilatasi (pelebaran) ruang jantung dan penipisan dinding ventrikel, tanpa disertai penyakit arteri koroner, penyakit jantung katup atau penyakit jantung lainnya. Tatalaksana kardiomiopati dilatatif memerlukan pemeriksaan lanjutan dengan menggunakan alat ekokardiografi, guna menilai fungsi sistolik dan fungsi diastolik ventrikel kiri. Pada penelitian ini menggunakan data sekunder dengan mengambil data dari berkas rekam medis RS. Jumlah pengumpulan data ekokardiografi terdiri dari 10 pasien dengan kardiomiopati dilatatif yang menjalani prosedur pemeriksaan ekokardiografi. Kemudian secara deskriptif dibandingkan fungsi diastolik LV pada kasus kasus dengan fungsi sistolik (Fraksi Ejeksi) LV <30% dan >30%. Hasil penelitian ini menunjukan pasien dengan EF ≥ 30% gangguan relaksasi (derajat 1) terdapat pada 60%,  20% memiliki gangguan compliance pseudonormal (derajat 2) dan 20%  lainnya dengan gangguan restriktif (derajat 3). Pada pasien dengan EF < 30% gangguan relaksasi (derajat 1) hanya terjadi pada 20% kasus, sedangkan gangguan restriktif (derajat 3) teriadi pada 80% kasus. Dapat disimpulkan bahwa pasien Kardiomiopati dilatatif dengan EF < 30% lebih banyak mengalami gangguan disfungsi diastolik restriktif (derajat 3-4), sedangkan pada pasien dengan EF ≥ 30% lebih banyak mengalami gangguan disfungsi diastolik gangguan relaksasi (derajat 1). Abstract

  Aortic regurgitation or aortic insufficiency is an abnormality in the aortic valve that becomes weak or enlarged so that the valve cannot close properly. It caused excessive amounts of blood in the left ventricle, which is received both from the left atrium and from the volume of aortic regurgitations through the incompetent aortic valve. It due to inadequacy of the aortic semilunar valve backflow of blood from the aorta into the left ventricle. Echocardiography is useful to determine cause of AR, by detecting the dilatation of the annulus and the aortic base, aortic dissection, or the primary of pathologic valve cusp. This case report aims to diagnose and determine the severity of Aortic Regurgitation with the PHT (Pressure Half Time) method on echocardiography. The results of the echocardiography examination showed an increased left ventricular thickness and mass accompanied by an increased left ventricular diastolic volume and pressure which can be called concentric LVH and aortic regurgitation. The results of this observation concluded that the techniques used in echocardiography have an effect on measuring the severity of aortic regurgitation. Abstract

Chronic Total Occlusion is usually found in CAD patients. Percutaneous Coronary Intervention (PCI) is good intervention in handling CTO. PCI is done in 30%  CTO patients with a success rate of 70%. Using primary case report with Descriptive cases, age 59 year with characteristics and CAD suspect. PCI was successfully done with predilatory insertion in LAD, no problems, no hemodynamic changes, no life-threatening ECG changes and no complications Keywords: Percutaneous Coronary Intervension, Chronic Total Occlusion, Left Anterior Descending

Abstract

The role of telemedicine in reducing morbidity, mortality and Coronary heart  disease has proven on a few qualified research, both in developing countries and in developed countries. by uploading electrocardiograph while coronary attack, like Acute myocardial Infarction to central data in the hospital, the patient or family can be quickly directed to the emergency room and at the same time operators, nurses and doctors at the hospital have prepared therapy and plans further medical help.
